To Schedule a Outpatient Physical Therapy Appointment
A written referral is needed from a physician, a physician assistant, or nurse practitioner in order to begin physical therapy. Insurance coverage for physical therapy may vary. Please contact your insurance carrier for further information. Appointments are available Monday through Friday, 8 am to 7:30 pm.
