| RADIOLOGY BILLING INFORMATION |
SEPARATE BILLS: You will receive two bills for radiology services. Charges for our physicians' service will be billed by our faculty practice, Stony Brook Radiology, P.C. If you have questions regarding your bill, please call our business office at 444-7611 or 444-3985. You will also receive a bill from University Hospital covering the use of its equipment, film, supplies and technical personnel. Questions about bills from University Hospital should be directed to the Hospital billing office at 444-4151. PRE-AUTHORIZATION NUMBER: Some insurance carriers require your physician to obtain a pre-authorization number for select radiology procedures such as MRI'S/MRA'S, CT scans and Nuclear Medicine exams. The pre-authorization number authorizes payment to the participating radiology facility. Please bring your pre-authorization number with you at the time of your appointment. REFERRAL FORM: Your insurance carrier may require that your primary care physician complete a referral form when authorizing radiology services. The referral form authorizes payment to the participating radiology facility. Please bring your referral form with you at the time of your appointment. SELF-PAY: Payment is requested at the time of service and may be cash, personal check or major credit card. MANAGED CARE CO-PAYMENTS: Managed care co-payments will be requested at the time of service and may be cash, personal check or major credit card. |
