Who Is Eligible?
All state hospital employees. (Research Foundation, CPMP, and Resident Physicians
are eligible but must process their forms through Human Resources Services,
West Campus.)
What Schools Are Covered?
SUNY schools (Stony Brook, Old Westbury, Farmingdale,
Empire State College and Downstate Medical Center).
What Is Covered?
A percentage of one course, to a maximum of three credits each Fall and Spring
semesters. Money is pro-rated to part time employees based on percentage of
work. The waiver is intended to be used for a second course, or a course not
covered by any other program.
Where Do I Get the Forms?
Human Resources, 3 Technology Drive, or the wall outside of Labor Relations, HSC, L-3,
or LISVH-Human Resources. Forms are available as of the Monday after July 4
for the Fall semester, and the Monday after Thanksgiving for the Spring semester.
What Is the Deadline?
Deadlines are posted on the instruction sheet of the waiver form.

Who Is Eligible?
All state hospital employees.
What Schools Are Covered?
All non-SUNY Schools, Suffolk Community College, C. W. Post, St. Joseph’s, etc.
What Is Covered?
$25 per credit to a fiscal year maximum of $300 based on funds availability within your department.

What Is the Deadline?
Ongoing. Paperwork should be started within the first 2-3 weeks of the semester.

PEF/PSTP
College Tuition Reimbusement (CTR) Program

All State Hospital Employees represented by PEF.
This program is not currently available during contract negotiations.

CTR Program at participating schools.
VALT Program for participating schools.

Who Is Eligible?
All state hospital employees represented by CSEA.
This program is not currently available during contract negotiations.
What Schools Are Covered?
Voucher program at participating schools identified by CSEA utilize a voucher
program. Non-participating schools utilize a reimbursement program.
What Is Covered?
The program offers three components: 1) Tuition vouchers 2) Tuition
reimbursement 3) Credit-by-Examination Fee Reimbursement.

1-800-253-4332 or 518-486-7814, option 2 for further information. Separate application forms must be submitted for each course.
What Is the Deadline?

Who Is Eligible?
Managerial/Confidential employees who have six (6) months continuous service
prior to beginning coursework, otherwise meet all program requirements, and be an M/C employee at course completion.
What Schools Are Covered?
All Schools.
What Is Covered?
Reimbursement is on the basis of charges for part-time study only. 75% of covered tuition is reimbursed, to a '07-'08 fiscal
year maximum reimbursement of $1,750.
Where Do I Get the Forms?
Guidelines and forms must be printed from the M/C website at www.goer.state.ny.us/mc/mctuition.html Please note: Part 1 Application Form MC009 must be signed by Shirley Menzies, Human Resources, per program guidelines.
What Is the Deadline?
Part 1 Application form is due to Shirley Menzies within 10 business days of
the first day of class. Part 2 Reimbursement request form is due at HR within
30 days of receipt of grades.

Who Is Eligible?
All state hospital employees represented by UUP.
What Schools Are Covered?
SUNY schools (Stony Brook, Old Westbury, Farmingdale,
Empire State College and Downstate Medical Center).
What Is Covered?
One course each Fall, Winter session, Spring, and up to two Summer semesters
as long as separate registrations are conducted and course
dates do not overlap. Waiver is on a "space-available" basis for tuition only.

What Is the Deadline?
Last business day before classes officially start according to School's academic calendar.
Deadline dates for various semesters are listed on the instruction sheet attached to the waiver form.
UUP Tuition Assistance Program
Instructions:
- Complete the UUP Tuition Assistance Program Eligibility Form. Please make sure that all information is filled in so that your approval is not jeopardized. If you have any questions, please call 444-4759.
- Have your supervisor sign in the Supervisor's Approval section.
- Return the original form to the Human Resources Office, 3 Technology Drive (Z=9300) for eligibility confirmation and signature. Faxes will not be accepted.
Once the form has been approved by Human Resources please pick up your form at Human Resources, 3 Technology Drive. You may pick up the form in 2 business days. You will receive the original form to submit to the Student Accounts Office, and a copy of the form is for your records.
If you have paid your bill prior to receiving the approved assistance, you should request a refund from the Bursar's Office.
