Download and complete the forms below as necessary:

• Change of Concentration Form
  Each student at Orientation completes a Declaration of Concentration Form. However, if a student wishes to change their concentration, they must complete a Change of Concentration Form and return it to the MPH Academic Coordinator for approval.

• Petition to Late Add/ Drop or Late Registration Activity
  Students wishing to add or drop a course after the University deadline must complete the petition and return it to the MPH Academic Coordinator for approval.

• Change of Enrollment Form
  Students wishing to withdraw or take a leave of absence must complete this form and return it to the MPH Academic Coordinator for approval.

• Student Term Activation Form
  Students wishing to return after a leave of absence (or a semester without coursework) must complete this form and return it to the MPH Academic Coordinator for approval.
