To apply for this position, please contact Diana Cohen at dcohen@numc.edu. Please note that applicants must be bilingual in Spanish and English. Below is a description of the Advocates for Community Health Program. Advocates for Community Health Health Leads is a national program that connects patients and their families to the community resources they need to be healthy, through trained student health advocates. Physicians prescribe basic solutions that will improve health, such as food, heat, housing, early intervention services, etc. – understanding that addressing non-medical but health critical conditions is an integral part of patient care. “Every day in America, doctors prescribe antibiotics to patients who have no food at home or are living in a car. Of course, medicine alone won’t solve these problems, and many patients will return with more serious – and more expensive – illnesses.” - HealthLeads We are opening a Health Leads desk here at Nassau University Medical Center in our Pediatric outpatient department to be staffed by trained student volunteers and interns from Hofstra who will help patients access community resources. This project is being implemented with community partners, as a collaboration between NuHealth, Hofstra through its Center for Civic Engagement, and Project DOCC. Student volunteers will become trained health advocates, connect patients to services, and educate NuHealth patients in advocacy skills. Our community-based collaborative, Advocates for Community Health, has partnered with Health Leads to pioneer a completely new way of delivering comprehensive health care to the community we serve. It will be launched on January 28, 2013. The NUMC/NuHealth Health Leads desk will be staffed by high achieving, highly motivated college students from Hofstra’s Center for Civic Engagement, who learn with and from families in the community about their health experiences and needs. The program empowers clinicians to “prescribe” services such as food stamp assistance or Medicaid enrollment. Health Leads’ Student Advocates use a robust resource database built together with community advisors and continuously updated to ensure accuracy to connect families with these resources. They then follow up with these families until the services are obtained, providing regular updates to clinicians. This collaborative process builds a community-based program that will be evaluated regularly by each of the partners, including families, university, medical center, and Health Leads to ensure that the following objectives are met:
