How to Earn CME Credit
At the end of each module, you will have the opportunity to register for CME credit. The total fee for this activity, including all four modules
and a total of eight credit hours, is $35. Your personal information will be used only for purposes of assigning CME credit, and will not be
sold or transferred to any other entity.
You need to score 100% on each quiz to earn credit. If you enter an incorrect answer, you will be directed to the workbook page containing the
correct answer. You can take as many tries as you need to get the answers right. You have one year from the date of registration to
complete the activity for credit.
When you have completed the quiz for each module and a brief evaluation of the activity, you will receive a certificate via email verifying credits
earned.
This online activity is approved for use from January 1, 2008 to December 31, 2011. Please note that registration after January 1, 2011, will
be active only until the end of that year.

sponsor continuing medical education for physicians.
Credit Hours
