Transition Services
The Cody Center is funded by OMRDD to provide Family Support services that focus on Transition to Adulthood for young persons with autism and other developmental disabilities. Susan Lesco will meet with families by appointment at the Port Jefferson Station clinical facility to discuss the services that the family will need to access and to assist the family with accessing government benefits and other programs. This service is available without charge by phoning 631-632-3070, option 3, and requesting a consultation appointment with Ms. Lesco.
The child/young adult must meet OMRDD eligibility criteria as follows:
Have a documented developmental disability AND a full-scale I.Q. of 60 or below; OR Have a documented developmental disability with a full scale I.Q. above 60 AND a Vineland Adaptive Behavior Scale 2nd Edition composite score under 70
The Center also sponsors a structured support and information group for the parents of young persons ages 14 to 24 in transition to adulthood. The next meeting of the group will be on May 6 from 7 to 9 pm at the Wang Center on the Stony Brook University campus. The support group presents an overview of transition including guardianship, OMRDD services, post-secondary education, vocational programs, and residential options. Neuropsychological testing and Vineland Adaptive Behavior Scale administration required to access OMRDD and other government benefits, or for admission to learning disabilities programs in college is available at the Cody Center. To schedule testing, please phone 631-632-3070, option 3.
